Nashville Hip Institute participates in a number of insurance plans. Plan types and policy provisions vary greatly from one policy to another. As the policyholder, it is critical that you are fully aware of the benefits available under your plan.
Before your appointment, please take time to familiarize yourself with your insurance coverage and be sure to bring your insurance card to each visit. If you are a member of a managed care plan that requires a referral from your primary care provider (PCP), we must have this authorization prior to your appointment. Our precertification specialist will contact your insurance company for authorization and benefit information for diagnostic testing, such as MRI or CT scans, and for surgery. We will submit claims forms on your behalf but are bound by the provisions of your policy. If you have any questions about the requirements of your specific plan, please contact your insurance company.
Please call our insurance office if you have questions concerning third-party insurance at (615) 284-5800
Payment will be due at the time of service. If you are unable to pay your balance in full, you will need to make prior arrangements with our team.
Nashville Hip Institute requests payment at the time of service unless we are a preferred provider for the insurance company or employer. Since most insurance companies do not reimburse the total amount due, you will be responsible for any deductibles, co-pay, or other related charges that are not covered under your plan. Contact our business office if you have any questions regarding your coverage or account balance at (615) 284-5800.
